Fotona Dynamis is a dual-wavelength laser platform. An erbium laser works at the surface, from gentle heating to full resurfacing, and a Nd:YAG laser works deep, tightening tissue, closing vessels and removing hair. Between them they cover laxity, texture, scars, redness, hair and nail fungus, and the combination protocol known as Fotona 4D. What it is

Two lasers, one platform, many modes.

The value of Fotona is range. The same device can deliver a no-downtime tightening pass, a fractional resurfacing that heals in days, or a full ablative peel, and can switch to a deep-heating wavelength for vessels, hair and tightening from below.

Er:YAG 2940 nm: the surface laserWater absorbs this wavelength so strongly that it removes skin a few microns at a time. In ablative mode it resurfaces precisely; in fractional mode it treats columns; in SMOOTH mode the pulse is stretched so it heats without removing anything, for tightening with no downtime.
Nd:YAG 1064 nm: the deep laserThis wavelength passes through the surface and heats the dermis and beneath it. In long pulses it tightens and closes vessels; in the very long PIANO pulse it heats the whole tissue thickness safely; in short pulses it removes hair and treats nail fungus.
Fotona 4DFour passes in one session: SMOOTH mode inside the mouth to tighten the lower face from within, FRAC3 for texture and small vessels, PIANO for deep tightening, and a light SupErficial peel for the surface. A non-surgical tightening and resurfacing protocol with a day or two of pinkness.

The clinic has several ways to resurface and several ways to tighten. Fotona overlaps with most of them, which is why the choice is made by the physician rather than by the brochure.

Erbium ablates with less heat than CO2, so it heals faster with less risk of pigment change, but tightens less. Fraxel is non-ablative and gentler again. Dr. Kuzel matches depth to damage.

Thermage is radiofrequency, one session, no downtime, and treats the eyelids. Fotona 4D is laser, a short course, a day or two of pinkness, and improves the surface at the same time.

Why a dermatologist

A versatile laser needs a considered operator.

A platform that can do many things can also do the wrong one. The judgement is in the mode, the depth and the patient.

Diagnosis firstTexture, redness and laxity all have causes that a laser does not treat. Dr. Kuzel examines the skin as a dermatologist and treats rosacea, melasma and precancer medically where that is what is needed.
Depth for the damageAblative erbium resurfacing is a real procedure with a week of healing. It is reserved for the skin that needs it, and lighter modes are used for everyone else. Overtreatment is the commonest laser error.
Skin typeThe Nd:YAG modes are among the safest lasers for darker skin. Ablative erbium carries a pigmentation risk in darker skin, and settings, preparation and candidacy are adjusted accordingly.
CoverageCosmetic Fotona treatments are not covered by Alberta Health Care. Assessment of a medical condition such as nail fungus or a vascular lesion is covered with a referral; the laser treatment is not.

Follow-upReview at four to six weeks for courses; at one week and three months after resurfacing.
Aftercare and downtime

From none to a week, by mode.

Downtime is set by how much of the surface is treated. SMOOTH and Nd:YAG modes leave the skin pink for hours; fractional modes for a few days; ablative resurfacing for a week.

After treatment

  • SMOOTH, PIANO, vessel and hair modes: pinkness and warmth for a few hours; normal activity the same day
  • Fotona 4D: pink and mildly swollen for one to two days; makeup from day two
  • Fractional erbium: redness and fine grid marks for three to five days; do not pick
  • Ablative resurfacing: raw, weeping skin for three to five days under ointment, then new pink skin for two to four weeks
  • Gentle cleanser and bland moisturiser; no active skincare until healed
  • Strict sun avoidance and daily SPF 50 for four weeks after any erbium treatment
  • Contact the clinic for blistering, spreading crusting, unusual pain, or signs of infection after resurfacing
  • Not used on a recent tan, in pregnancy, or with isotretinoin in the past six months for resurfacing

What to expect

Expected outcomes.

Tightening from Fotona 4D builds over three to six months as collagen forms and is assessed after the course; the result is firmer skin and a cleaner jawline in the same modest range as other non-surgical tightening, maintained with a session a year. Ablative resurfacing produces the largest single-session change in texture and wrinkles, visible once the skin has healed and improving for three months.

Vessels close within one or two sessions. Hair reduces progressively over a course of six to eight sessions. Nail fungus clears slowly as the nail grows out over a year, with a course of laser sessions along the way and no guarantee of cure.

Fotona does not replace surgery for heavy laxity, does not treat brown pigment as well as a pigment laser, and does not treat fine facial redness as selectively as VBeam. Dr. Kuzel says which tool is right at consultation.

What is Fotona 4D?

A four-pass protocol in one session: tightening from inside the mouth, a fractional pass for texture, a deep heating pass, and a light surface peel. It tightens and refreshes the lower face with a day or two of pinkness, over a course of two to four sessions.

Fotona 4D or Thermage?

Thermage is one session with no downtime and treats the eyelids. Fotona 4D is a short course with a day or two of pinkness and improves the surface at the same time. Dr. Kuzel recommends by what needs treating and the downtime you will accept.

How much downtime is there?

It depends on the mode. Tightening, vessel and hair modes leave a few hours of pinkness. Fractional modes leave a few days. Full ablative resurfacing needs a week of healing and several weeks of pinkness.

Does it hurt?

Non-ablative modes feel like warmth and need no anaesthetic. Fractional and ablative modes are performed under numbing cream and feel like heat and prickling.

Is Fotona safe for darker skin?

The Nd:YAG modes are among the safest lasers for darker skin and are the preferred hair removal wavelength for it. Ablative erbium carries a pigmentation risk in darker skin and is used with adjusted settings and preparation, or avoided in favour of gentler modes.

Erbium or CO2 for resurfacing?

Erbium removes skin with less heat, so it heals faster with less risk of pigment change but tightens less. CO2 heats more and tightens more with longer healing. Dr. Kuzel chooses by the depth of damage and the skin type.

Can it treat nail fungus?
